I'd better have collected all the dog tags (think I have) because I don't wanna have to go through on Extreme again. :-D
I'm at the bit just before the Ray fight. The part where you team up with snake and kill about a thousand soldiers. Snake drops you rations (which is lucky because without rations it would be impossible) but he doesn't use any himself. And he doesn't hold back and kill from a distance either. He just runs up to a soldier and then pumps him full of bullets. Then moves onto the next. Thing is, he always takes a few bullets himself as he is running.
I think I was quite close to doing it once (depending on how many men there are at the end of course - and this is only the first room!) but I accidentally fired a grenade into Snake's face!
I was in first person view, aiming my grenade launcher when I saw someone right next to me. Thinking it was a bad guy, I fired. It wasn't a bad guy however, it was Snake. Took away most of his hits. When he was on the floor, the soldiers finished him off. DAMMIT!!
I think I'll go check if I've got all the dog tags.
And by the way, it is Hideo Kojima you get off Snake in the Ascending Colon.

This one`s got me as well. I keep missing several tags in

Hold 3, its a gaurd in the middle at the front.
Hold 1, its a guy in the middle but in the second row, but a lot of those surrounding him to sleep, then make some noise!

Errrr......*blushes*......actually.....I haven't completed it on Extreme. :-D I can't remember where I heard that Kojima thing but I'm sure it's true.
However, hold that applause! It shouldn't be much longer now. I am on Vamp and he shouldn't be too much trouble. After which, I think it is just the bit where you team up with Snake that might be tricky. Solidus is always easy and I'm hoping the Rays won't cause too much trouble. Mind you, I said that about the Harrier. :-D
Anyway, here is how to defeat Olga and get the last tags in the holds. When you start on Olga, duck down and there is a hole beneath some of the crates. Get to a postition where you can see her foot when she stands by the locker type thing on the left. Now as soon as you see her leg, shoot her. She will run to the right, but then run straight back. Shoot her leg again, and she will do the same. She will then get stuck in the same continuous cycle and as long as you don't miss a shot then you'll be alright.
As for the last dog tags, when you enter Hold 1, go down the first ladder. Before you go down the second one, jump over the railing and shuffle across to the far end (you'll need full hits or level 2 grip to make it). When you get to the end, flip over and there will be a guard. He has the first dog tag. Then use the door to enter Hold 2. Once in Hold 2, jump over the railing again and shuffle across to the far end just like before. There is no guard here so go through the door to Hold 3. As soon as you enter Hold 3, there is a guard facing the opposite direction. He has the second dog tag.

I can't imagine why you would be able to either. It's understandable why he might have Meryl's, Liquid's, Iriquois's and his own but he couldn't have Solidus's.
I know he couldn't have Kojima's either but that's besides the point! :-D
