What a great game it was.
Carmageddon 2 had almost unprecedented amounts of carnage.
Soldier of Fortune allowed you to blow peoples legs off and watch them screaming on the floor.
Vice City, although not that gory, allowed you to be completely morally bankrupt.
Mortal Kombat was the first game that made me think 'Jesus, did he just rip that guys head off?'
The latter title would have to go to Mortal Kombat, after it's release caused mass media hysteria on an unprecedented level. Carmageddon was bad, but MK was much more explicit.
Most gruesome ever? I'm not sure. Perhaps that 3D0 murder mystery game (or was it on the jaguar?) which included photo scenes of such things as a dismbodied skull & spinal cord. Don't remember the name of it now though.
