The people monitoring David will be checking his urine to make sure that things are ok.

So by seeing him sitting staring blankly into space or with his eyes closed, you'll be able to tell he's seeing little animals?
Considering those are basically his only options throughout the whole thing, I doubt you'll notice much difference other than he looks slightly more ill and thin after a while.
