Because it's fun simply put.
Theres nothing better than watching your mate charging into a pack of impossible to kill bulls and being knocked 100 feet through the air to his death. With lives, people would be more cautious and SS is not the kind of game people should be playing cautiously on.

Infinite lives sounds more like Speshul Sam than Serious.

Well, you still die with infinite lives. The fun comes from surviving a particularly nasty wave. With invinvibility, you could just stand there and punch for half an hour.
Demo and a few mates for a single level yes. Full game? nope. I wouldn't.

Yes, hence the: 'one-step from'.
