I've never seen it, but I believe the worst scenes in film are in a french film called 'Irreversible'.
Out of those that I've seen :
Gangster No.1 - The First Person scene where the gangster attacks the guy with an axe - probably the one that makes me wince the most
Goodfellas - the stabbing scene in the boot of the car
The Thing - Most of the transformations in that are not pretty
