In my opinion? It's because the west approve of leaders who will tow the line and keep the country in a state of poverty whilst their resources are exploited by western nations. Those 3rd world leaders get rich, and the west is kept happy. Who cares about a few billion starving? As evidence of that, look at what happens to the few 3rd world leaders who actually seem to give a damn about their people. Allende ended up dead in a CIA sponsored coup. Contrast that to those 3rd world leaders who are corrupt, brutal psychos; as long as they tow the line, they have long and happy lives.
Obviously, I'm oversimplifying but that's it in a nutshell.

Because doing something that is so blatantly about keeping the third world in virtual slavery to the west would cause revolution the world over. One lesson that the World Bank and IMF have definitely learned is that it's better to shear sheep and not rip off their skin in one go.
