You'll ave to use thermal paste to ensure a good connection between the blocks and the GPU. It is quite fiddly and requires a steady hand, but once screwed in (the blocks) to the card, you just have to position the main heat spreader across the block (again applying paste) and it'll work.
It works by basically acting has one huge heatsink; you can tell by the surface area of the heat spreader.
