I am looking for something to significantly cool my graphics card down as I am getting frequent crashes after a while due to the extreme heat and poor air flow in my case. Would this do the trick?

You'll ave to use thermal paste to ensure a good connection between the blocks and the GPU. It is quite fiddly and requires a steady hand, but once screwed in (the blocks) to the card, you just have to position the main heat spreader across the block (again applying paste) and it'll work.

It works by basically acting has one huge heatsink; you can tell by the surface area of the heat spreader.

I have managed to find some information but it doesn't say anything about fitting to an 9800 XT but refers to this fitting on standard graphics cards.

Does the top one need any modding to the fan? The one I wanted involves removing the fan and messsing a round with the card too much. Also how does it work? I can't find information about it.

Any information would be appreciated, cheers.
