A guy at work has been off ill with some virus for a number of weeks.

His other half also works here.

She walked into the office earlier, and someone asked: "How is ..."

To which she replied: "He's fine, thanks."

Am I wrong, then, to wonder: why isn't he at work?

One of them has had an affair, and they're in the process of splitting up.

Either that or he has an terminal disease.

Because she has no interest at all in the person that asked her that, either:

Because she doesn't really like him/her [the one that asked].

Or she thinks they don't really care, so isn't paying attention.
