So if he took the test or not, they would have known he was the heir (despite the obvious complications of being dead if he did take it)?
Would he have definately died if he took the test?

No.
In fact no one was certain that the heir would pass up on taking the test. He might not of had the illness, and if he had have been able to take this testm it would also have shown he was the heir.
