My twelfth is in phantom and spectre and haste
My whole stalks the night in a quiet dreamlike state
With no memories the next day of being out late

Bang on. Although the indirectly should really be directly (i think). It didn't kill him, but he wouldn't have died if it hadn't fallen through the gate.

And the closest people have come is Ineedsleep's bungie jumping. Although that wasn't THAT close.
