These are all mistakes of formal ways of questions, here are hte less formal ways or "street-talk" kinda thing.


To ask someones name its:
Was ist Ihr Name?

To ask there age its:
Sind Sie wie alt?

To say your name its:
Mein Name ist Devious Dragon
