I think it's partly the realisation that what he is witnessing is a portal, as the actual travelling through it.. Or he creates it or something. But what he ends up in is a tangent universe rather than the one that's just been destroyed... And everything that happens during the "Mad World" song shows that people in this tangent universe have a slight knowledge of what's happened in the other one - Frank touches his eye, other people do other things, which they wouldn't have known about otherwise.
Confusing? Yes.
Donnie's role is to sacrifice himself so that the events that happened in his universe don't happen in that one. I think.
Sight and Sound reckoned it was probably much simpler to start off with, but an editor suggested making it weirder to go for a more culty feel.
