Been playing Warcraft 3 and The Thing lately (finally got them working) but I played a bit of Mafia before, and I'm up to the bank raid level. Went in, got the keys from the manager, went down... and can't get through the door. There's two metal doors, one doesn't do anything, and the other refuses to open. So what the heck am I meant to do? Is it meant to be like that, or is there something horribly wrong?

What, so the door that doesn't open needs those keys by the teller? Ah. I looked for them, but couldn't see them.

There are two metal gates, and the one straight in front of you needs to be used, You can also start shooting the guards behind the gate on the left, and one of them may come running to open the other door. Sometimes the game can glitch and you can't perform actions properly.
Also, have you got the keys from behind the cashier in the main area, where paulie is. They are hanging up behind the cowering tiller.
