£15 Card, all set up yesterday and downloaded over 100mb in 2 hours last night, I'm well pleased.
As you only have a 128k connection, a USB card would be fine. The engineer told me that they only recommend an Ethernet card for 512k and upwards. But if you think you might upgrade later, you may as well go for the Ethernet option now - saves you spending more later!
As for cost - I have 512k from NTL, and it costs £24.99 per month.

By the way this offer only applies to certain NTL customers before you all bombard NTL.

When I arranged for my connection, I (unusually!) had a very helpful engineer. He advised me to go for the 10/100 network card option because (and I quote), "while it shouldn't really matter, in our experience the network cards give a better performance in terms of over-all speed".
So go for the 10/100 card. I have a 3Com 3C SOHO Fast Ethernet adapter, which was easy to install and completely hassle-free. And under WinXP, it gets configured perfectly under Plug & Play without me needing to install any drivers whatsoever.
