Frantic Flea on the SNES springs to mind. Surely one of the worst ever.
Dave Mirra 2 is such a bad game.. The levels are poorly designed, as is the overall play of the game. The handling's terrible and it moves at a snail's pace. Long live Tony Hawk.
The worst game that I have ever played is probably Wetrix.
They were, because I paused the game
Controller probably wasn't plugged in.

The worts games I have played so far have been:
PC - Army Men: Toys in Space, I don't know what the hell I was thinking when I bought this pile of crap, just awful. I've since thrown it away thankfully.
PS1 - Chase The Express, I bought this because it looked like a good action game, and it's one title I really should have read reveiws on before hand. It was way too easy, I could complete the damn thing in less than a day, plus it wasn't really much fun either. I've now sold it.
PS2 - Red Faction, An overrated game in the extreme, it has nothing new to show ecept the quite naff Geo-mod engine, pathetically easy enemies to kill, over powered weapons, dull level design and it's far too easy for a FPS. Hope the sequel is better than this tat.
GC - None so far, I only have 4 games at the mo, and I thing they are all great, but it's only a matter of time before I pick up a real stinker.
