You need to use a 'smash' attack to get it anywhere. Push accross quickly and hit A at the same time.
ahem... I can get 20.5m!
Easy, use DR Mario
Pick up the bat then run at the bag hit it 4 times with your cape then throw 4 pills at it then run in again and dash it with your cape until the time runs down, should be able to get about 4 hits. Then smash it with the bat making sure you hit it with the tip of the bat and whooooooomph 4 million miles it goes.
Well not quite 4 million but you get the idea.
