Personally, I detest happy endings, unless it has traces of bad in it, like the ending to both Metal Gear games, there is some tragedy/controversy around it, letting you know that everything is not well. After all, I'd rather play as a terrorist than the good guy, but that's just me ;D

What if it's a game like Bishi Bashi Special or Fuzion Frenzy? Do you expect people to be killed in that. I find fighting games (boxing, beat 'em up, wrestling) to be very engrossing, yet nobody dies in them, do they (well, we can make allowances ;D)? I don't think it's neccesary in all games, but can be a nice touch some of the time.
2. For the emotion, that ONE person must be a female.
I agree here, that a female adds some depth and emotion to the game, but is also not neccesary. You don't need a female to go and blow holes through walls, but as I said, it adds to depth and deepens the storyline and how you approach the game.
3. The person who has just been killed has to be shot in the head with a machine gun several times.

Excellent point. It's annoying in so many of the best games to see someone die, go around a corner, turn back and see the body gone. I'm glad to see that newer games now have implemented this feature, like Red Faction (thank you SR! :D).
