I'm sure we all have our own gaming ideas that would make our games seem that little bit harder.
Some examples of a better gameplay structure could be
Real damage to your character, eg if your guy gets his right arm shot and (presuming he's right handed) finds it difficult to use his left hand to fire, affecting aim and recoil, or he can't use big weapons like a rocket launcher which require 2 arms. Also, a hit to the head with a baseball bat would give messed up controls making it difficult to know where you are going, just like if you were hooked in the face in real life.
Another one could be if you get shot in the leg you start limping, slowing you down and making you easier to hit, it would make you think twice about picking up your pistols and taking on a guy with two miniguns in timesplitters.
In driving games, I'm not really sure if this does happen, but instead of just having realistic damage graphics, getting a hit from the back sends your driver forward, maybe hurting his head and blurring his vision etc, or maybe bursting his tyre, wrecking his wheel, and having toooo get it replaced.
Extreme Sports Games - they fall from enourmous heights yet they get up and skate, cycle, board etc away, without so much as a bruise or a cut on themselves. They should lose health and hurt themselves making it more difficult to perform tricks and get speed etc.
In other words I think that games should have realistic damage.

Game developers are leaving things liekthis out becuse it makes games so much harder but they are much easier now than ever because of the addition of memory cards etc. so these developers need to make games harder by adding more realistic gameplay. People always mention how a game is realistic but if it doesn't include gameplay features such as these then they'll never be realistic.

It would get annoying if you lost control of your character if you got hit, Id end up puttin the controller thru the screen.

I have anger managment classes every tuesday at 7:30, dont seem to work tho..... :-P

i don't think games would be made harder by the fact that your character becomes harder to control if he gets hit, i just think the game would become more frustrating. ideally, i'm sure developers try to steer clear of that because playing a game should be fun, not a chore.

i'm all for more realistic damage but i'm a bit skeptical about it affecting the gameplay....still, nice post.
