For the last part of mail(), it needs to be "From: [email protected]", NOT just "[email protected]".

Oh hang on, the one I sent you is where users fill in a Mail Form, and send it back to you, is that what you wanted?
I've sent it, and it should work once you change the Variables. If Not, let me know where it's faulty.
