Thats correct. You have to buy an Action Replay. Its an accessory for the console. You boot up the Action replay and enter the cheats into its menu, then boot up the game and the cheats will be active.
There are loads of regular cheats for vice city so try those before spending any money.

An "action replay" is a device you can but for your PS2 so that you can cheat in ways not supposed to be used.
I don't know much about them, but pb does.
I think its sort of like a patch that someone made, like using Seifer and Edea all the time in FF8.
