Nothing is more annoying than finding all the info off the site, then trying to squeeze it into the stupidly small order form legibily (sp), just so you can send a cheque/PO. Usually you have to scribble it all out and write it on another piece of paper, which can get messed up. So:
How about, when you go to the checkout, you can fill in your details and press "Printable form", and you get a simple page with a form to print out. No mess, no mistakes. Sorted.
