I normally just put my windows xp disk in my cd drive and then restart the computer and boot of the cd drive, then follow the instructions and at one point it will ask you if you want to format the hard drive. then just let it do it's thing and it will install windows right afterwards.

Older windows Start>Run>"command">fdisk then follow the instructions.
For XP you just put the disk in like previously said.

Yes.

Formatting your computer means basically deleting everything.
