I think the last World Cup Winners, the last Euro winners and the AFrican cup winners are always invited. Plus Brazil do it anyway, because their competitions and their leagues competitions stop around the time of the confederations cup.

I always thought it was the world cup 'warm up' for the country who's hosting it. There are a few teams that are always in, others get 'invites (either Japan or the US always get invited, I forget which).
So it's a choice thing?

Yes.
It's an invitational tournament I think. Usually only France and Brazil are the big teams that play for it in real life I think. It's 1 or 2 teams per region (CONCACAF, etc) I think.
