Your IP Address goes along with any HTTP request you make, be it for a web page in your browser's address bar, clicking on a link or submitting a form. Otherwise, the web server wouldn't know where to send the response. It's how the web works.

In it's basic form, no. The IP will refer simply to the computer to which it has been assigned. If, however, you download a virus or are a victim of hacking, then it may be possible to steal any information that is stored on the computer.
It is also possible to trace people via IP addresses, but only if the big wigs like your ISP and BT are willing to cooperate.
