It should be as geeky as possible.

Find any old picture, then go to Options in CS. There'll be a button saying "Import Spray", click, find your picture in the directory thing, and upload.
I don't think there's any limit to size or anything, it just shrinks it to fit.

Use Fireworks MX or Photoshop or whatever - some form of image editing program.
Once you've made it and stuff, in game go to Options > Import spray, and select the image.
