I think it's partly that the teachers at a primary level (where I had most of my experience) aren't critical enough of kids because they worry too much that I might reduce their love of writing.
This carries on in to secondary schooling and the older children find it harder to change their habits.
I'm sure I was as guilty as anyone of repeating words far too much in my secondary classes. 'Then' 'and' and 'however' are three common repetitions.
Using another word is only one way to get around repeating the same word again. Another way is to restructure your sentence.
